I think I can can provide more details, I had a very similar thing happen. I logged back in and one of my ships was missing, I spotted it sailing off into oblivion at full reverse thrust. Had to grab a fast little ship and put on a velocity security bypass to catch up to it so I could order the captain to follow me.
